DO YOU TAKE INSURANCE?
I do not accept insurance and am considered an out-of-network provider. I can provide you with a health insurance claim form which you can submit to your insurance company for possible reimbursement or credit towards your deductible. Payment must be submitted to me before I can compile a health insurance claim form for you.
I do not accept Medicare and therefore I cannot, by law, treat a Medicare beneficiary for normally covered services. However, I can see a Medicare beneficiary for a wellness visit or a sports/movement-specific bio-mechanical analysis. Please contact me for more information regarding whether or not it would be appropriate for you to see me.
WHAT FORMS OF PAYMENT DO YOU ACCEPT?
I accept All major credit and debit cards, cash, checks, direct deposit, venmo and paypal. You can also use your health savings account funds or flexed spending card to pay for your visit.
WHAT TYPES OF INJURIES DO YOU TREAT?
The satisfying aspect of cash-based physical therapy is my patient demographic is very diverse. As long as I deem it appropriate, I can help with everything from a person with a neurological condition to an elite athlete with a sports injury. My focus with every client is ultimately the same: What's the goal? What's stopping the goal from happening? And what do we need to do to change that? Occasionally, there will be times when I may refer a client to a Medical Doctor, another healthcare practitioner, movement practitioner or body worker for services I cannot provide.
DO I NEED A PRESCRIPTION FROM DOCTOR TO SEE YOU?
Utah is a direct-access state and allows people the freedom to see a physical therapist without a prescription from a doctor. However, your insurance company may require a doctor's prescription in order for you to get reimbursed. Please check with your insurance company regarding this policy.
